Second Opinion Only
The Second Opinion Service provides an expert, records-based medical opinion intended to help you better understand your condition and prepare questions for your treating clinician(s). It is not a substitute for in-person medical evaluation, diagnosis, or treatment.

No Physician–Patient Relationship
Use of this service does not create a physician–patient relationship between you and Dr. Ramesh Babu Pitti, M.D., Ramesh Babu Pitti, MD PC, or any affiliated clinicians, employees, contractors, or agents.

Records-Based Review (No Physical Exam)
Dr. Babu’s opinion is based solely on the information you submit. No physical exam is performed, and Dr. Babu will not have access to your full medical history unless you provide it. A records-only review has limitations and may not identify all conditions or risks that could be found through an in-person evaluation or additional testing.

Not Medical Advice; No Treatment
This service does not provide emergency care and does not include prescribing medications, refilling prescriptions, providing controlled substances, ordering tests, interpreting new studies outside the submitted materials, providing disability determinations, or establishing ongoing care. Any decisions about diagnosis and treatment should be made by you and your treating clinician(s).

No Guarantees

Your Responsibility for Accurate Information
You represent and warrant that the information and records you submit are accurate, complete, and belong to you (or that you are legally authorized to submit them on behalf of the patient). You are responsible for ensuring records are legible and complete. Dr. Babu’s opinion may change if additional or different information becomes available.

Eligibility & Location
This service may not be available in all jurisdictions. By using the service, you confirm you are permitted to receive this type of records-based medical opinion where you live and that you are not seeking emergency care.

Privacy
Information you submit will be treated as confidential and used only to evaluate your case, communicate with you about your submission, and provide administrative support for this service, consistent with applicable privacy practices.

Fees, Refunds, and Process
Fee: The Second Opinion Service fee is $500 and covers physician review time and administrative handling.
Refund Policy: Once Dr. Babu’s review has started, the fee is generally non-refundable. If your submission is insufficient to provide a meaningful opinion, you may be contacted for additional records. If you choose not to provide requested materials, a refund may not be available.
Turnaround Time: Turnaround time may vary depending on record completeness and clinical complexity. You will be notified if additional information is needed.

Assumption of Risk
You acknowledge and agree that a records-only review has inherent limitations and may miss information that could be identified through an in-person examination or additional testing.

Release; Hold Harmless; Indemnification
To the fullest extent permitted by law, you agree to release, indemnify, defend, and hold harmless Dr. Ramesh Babu Pitti, M.D., Ramesh Babu Pitti, MD PC, and their employees, contractors, and agents from and against any and all claims, demands, damages, losses, liabilities, costs, and expenses (including reasonable attorneys’ fees) arising out of or related to: (a) your use of the Second Opinion Service, (b) any inaccuracies, omissions, or illegibility in the materials you submitted, or (c) actions you take or do not take based on the opinion provided.

Limitation of Liability
To the fullest extent permitted by law, in no event will the total liability of Dr. Babu or his practice for any claim arising out of or relating to this service exceed the amount you paid for the service ($500).
